Steps:
Create a refresh button
Create a div/span with id for captcha image
span id="Captcha-image"
?php //echo $captcha_image;?
/span

Create a function (getNewCaptcha) in script , call ajax in that
After that pass the controller function you are creating captcha in.
Last show that in id for image
function getNewCaptcha() {

$.ajax({

{
$('#Captcha-image').html(response);

}
